 Dad talked to a friend and he said he had a similar problem and here is what he said......

Wrong lug nuts. A slightly different thread pitch is all it takes. The nuts will feel like they are tightening up with an impact wrench but then you can pull them right off with your fingers.  Luckily my "learning experience" with that didn't involve a wheel coming off.
